Cyclic amp glucose8/9/2023 ![]() This has been challenged recently in experiments utilizing livers and primary hepatocytes lacking either AMPK or its upstream activating enzyme LKB1 4. Metformin inhibits mitochondrial respiratory Complex 1, thus reducing hepatic energy charge ten years ago it was suggested that metformin functioned through activation of the kinase AMPK 7– 10. Despite the wide acceptance of metformin as a first line therapeutic for diabetes, its mechanism of action remains unclear. These data support a mechanism of action for metformin involving antagonism of glucagon and suggest an approach to the development of antidiabetic drugs.īiguanides exert their major effect through inhibition of liver glucose production, though enhanced glucose disposal has been noted in some studies 5, 6. Metformin leads to the accumulation of AMP and related nucleotides, which inhibit adenylate cyclase, reduce levels of cyclic AMP and protein kinases A (PKA) activity, abrogate phosphorylation of critical protein targets of PKA, and block glucagon-dependent glucose output from hepatocytes. Here we provide a novel mechanism by which metformin antagonizes the action of glucagon, thus reducing fasting glucose levels. The suggestion a decade ago that metformin reduces glucose synthesis via activation of the enzyme AMP-activated protein kinase (AMPK) has recently been seriously challenged in genetic loss of function experiments 4. Nonetheless, the mechanism of action of biguanides remains imperfectly understood. For fifty years, one of the few classes of therapeutics effective in reducing glucose production has been the biguanides, which include phenformin and metformin, the latter the most frequently prescribed drug for T2DM 3. The inability of insulin to suppress hepatic glucose output is a major etiological factor in the hyperglycemia of type 2 diabetes mellitus (T2DM) and other diseases of insulin resistance 1, 2. ![]() Glucose production by the liver is essential to providing substrate to the brain during fasting. ![]()

Packrat pance8/9/2023 ![]() This became problematic for me, as I read the vignettes and prepared an answer (typically diagnosis) in my mind, then got to the end of the question and answer choices and they were asking from something entirely different. It might be asking for which medication is CONTRAINDICATED. It might be asking for the GOLD STANDARD diagnostic tool. Rarely is the question asking for the actual diagnosis (that would be way too easy)! It might be asking you for the INITIAL STEP in management. With limited time, it is critical to understand what the question is actually asking you. (By the way, these are my best exam-taking strategies for any PA school exam, not just the PACKRAT!) Read the END of the question first. It’s important to know your strengths (and more importantly) your weaknesses so that you can improve before the real deal, make or break, board exam. Of course, the PACKRAT is typically not taken for a grade or anything other than your own personal knowledge but I like to take practice exams as if they were the real thing. Now that you know what to expect, let’s talk about strategies for taking the exam. *The time you have to take your exam is adjustable between 4 and 6 hours depending on your proctor. The breakdown of topics is similar to the PANCE with cardio and pulmo being the “heavy hitters.” I’ll list the actual percentages of each topic below. There is a list of normal lab value reference ranges to the side that you can open at any time. ![]() That math breaks down to roughly 1 minute and a few seconds per question. It is a totally computerized exam consisting of 225 multiple choice questions and you have 4 hours* to take it with no built-in breaks. ![]() Also, after posting about PACKRAT day on my Instagram story, I received multiple questions about it so I thought it would make for a good blog article!įirst let’s talk about the basic set-up of the exam. There are endless resources and blog articles regarding the PANCE but not much about the PACKRAT. ![]() Every program is a little different but at UAB, we take it at the end of didactic year and again at the end of clinical year before taking the boards.īefore taking the exam, I did some googling for information about it but did not find much. My classmates and I are nearing the end of didactic year (WOOHOO) and recently took the PACKRAT exam which is a “practice PANCE” that PA students can take to predict how they will perform when taking the boards. ![]()

Mongodb speed vs mysql8/8/2023 ![]() For example, you can bring Salesforce data to MongoDB and learn more about your e-commerce and customer service processes. The two integrate easily to give you more control over your data, allowing you to move data to and from MongoDB. You can also use Integrate.io to simplify the MongoDB ETL process. If a vendor doesn’t provide you with superb client services, then you can search for an alternative to help make your database more efficient and flexible. MongoDB doesn’t force you into a vendor lock-in, which gives you the opportunity to improve its performance. ![]() Depending on the types of data that you collect, you may benefit significantly from this feature. MongoDB says it’s significantly faster than MySQL for inserting and updating a vast number of records, while MySQL is faster at selecting a high number of records. In the MySQL vs. MongoDB speed debate, MongoDB usually comes out as the winner. MongoDB can store large amounts of unstructured data much faster than MySQL (and other databases like PostgreSQL) thanks to slave replication and master replication. For example, you can extract RDBMS data, transform it into the correct format for MongoDB, and load it into the database. You can improve MongoDB vs MySQL performance by integrating either database with an ETL tool that can make data storage and manipulation in real-time easier. MySQL performs well as an open-source relational database with multiple tables. However you plan to use MySQL, the database’s performance gets a boost from full-text indexes, a high-speed transactional system, and memory caches that prevent you from losing crucial information or work. MongoDB offers multiple ways to create queries. MongoDB also gets performance praise for its ability to handle large amounts of unstructured data. When it comes to MongoDB vs MySQL performance, it's important to consider how each database would affect your projects. Difficulties with some complex data types.Limitations on common table expressions. No Java or Python integration or embedding.Of course, even people who enjoy using MySQL note the features they don’t like. Some of the positive features that users frequently mention include MySQL’s: The MySQL database also enjoys a high rating on TrustRadius, with the same score as MongoDB - 8.6/10 (February 2023). Occasional slowness in the cloud environment.Though many reviews about MongoDB are good, some users do report some negatives, such as: MongoDB supports a range of indexing methods.Easy query language for developers (MongoDB Query Language).A flexible schema for altering data elements. Some of the things that authentic MongoDB users say they like about the database are: Reviewers on TrustRadius give the document-oriented database a high rating of 8.6/10 (as of February 2023). Maya oakley illness8/8/2023 ![]() ![]() Asked if being a veterinarian assistant was the only job available for her in the Yukon, Maya laughs and says there were plenty of other options but none that gave her the opportunities to do such important work and get to travel.Įach of the Oakley daughters have enough hands-on experience to give them an edge in the vet world. Maya has more of an interest in a criminology degree but when she’s home from school, she’s returns to the wild to work alongside her mother. Her plans are to attend veterinary school. The time working with her mother has rubbed off on the 21-year-old Sierra who is a senior at St Francis Xavier University in Nova Scotia where she is pursuing a pre-vet degree. Even my dad gets roped into helping,” Sierra says. “One way she does that is by incorporating us into the vet work. She’s watched her mom find the balance between taking care of the wildlife and still finding time to be a mom. Sierra says her mother’s work would be enough to make her seem like one of the toughest people in the Yukon but that’s only part of the challenge. “The neighbors all knew I was a huge animal lover and if there was an injured rabbit, they would bring it to me.” We lived near a creek and so all of my time was either looking out the window or playing by the creek where I could watch the raccoons, I would try to bait the raccoons into the house. “My grandmother would feed all of the stray cats in the alley. “Animals were always part of our family because my parents are huge animal lovers,” Oakley says. ![]() Oakley’s journey to become veterinarian started long before she took her first class. After graduating from Atlantic Veterinary College in 2000 with a doctorate in veterinary medicine, she worked for nine years as the wildlife veterinarian and regional biologist for Yukon’s Fish & Wildlife department. It was during that trip that she met and fell in love with local firefighter Shane Oakley. The Indiana native made her first trip to the Yukon as part of an ecology study while she was working on her undergraduate degree in zoology at the University of Michigan. The work she has done through the clinic she runs out of her home in southwest Yukon – with the help of her three daughters Sierra, Maya and Willow – plus running her own mixed practice throughout Northern Canada and Alaska has become the fodder for the two television series. She’s powered her way to a practice that covers hundreds of miles in the Yukon Territory in Northern Canada. “The main thing is to not let anything get in your way and power through it for the animals that need you.” “Nothing is designed for dealing with wildlife and you just sort of have to figure it out,” Oakley says. She feels more comfortable with terms like adaptable and innovative. Oakley, Yukon Vet,” as well as with the NBC Saturday morning series “Wilderness Vet,” has earned that description because she will have a day where she goes from tending to a sick bear to hanging out of a helicopter door (despite having a dislike for flying) to shooting a dart gun. 60 in 1 arkanoid spinner8/8/2023 ![]() ![]() When pulled through the water at almost any speed, the spoon attached to the front of the weight will spin quickly around the lure like a helicopter blade, hence the name. Spinners are not usually designed to look like anything per se, but when fished normally they probably imitate a minnow. Spinners come in many different sizes, colors, and dressings. Very often the hook itself will be dressed with some kind of hair or feathers to disguise it. It typically consists of a swiveling side-mounted spoon at the front of the lure, a weighted midsection, and a treble hook dangling from the back of the weight. These integrated spinners usually have a small range of disctinct positions ( Ikari Warriors has a 12 position one), and are unique to the game that uses them.Ī spinner is a time-tested and very effective type of freshwater fishing lure. Finally, some games (such as Ikari Warriors), integrate spinners into their joysticks. A few other titles use microswitch based spinners that only have a few distinct positions. Discs of Tron uses an optical spinner that can be pressed down or pulled up for an additional degree of control. Unique spinnersĪ few games use other kinds of spinners. These spinners are not as reliable, but are very cheap to repair when they do fail. These provide true analog movement, but do not rotate nearly as freely as optical spinners (nor do the provide for full 360 degree rotation). These spinners work by turning a POT underneath the control panel. Analog spinnersĪnalog spinners were mostly used in 1970s era ball and paddle games, such as Pong, Winner, and Breakout. Games such as Arkanoid, Tempest, and Homo use optical spinners. These are digital in nature, but they closely mimic analog operation. Turning the spinner turns a small encoder wheel which translates into movement inside the game. They work on the same principle as the mouse on your computer. Optical spinners are by far the most common type. I will also touch on a few unique types of spinners that are very uncommon. I am going to talk about optical spinners first, as they are the most common. There are two basic variety of spinners, optical and analog. Spinner games dropped a bit in popularity in the early 1980s, but enjoyed a brief surge when Arkanoid was released (Arkanoid was probably the best spinner game ever made). ![]() The first spinner controllers were used in games like Pong, Breakout, and Super Breakout (and their many clones). They were also used on several early console systems (like the Atari 2600), but for some reason Atari called them " Paddles".Ī spinner controller is a knob that you rotate left or right to cause movement on the game's playfield. Michael jordan jimmy butler8/8/2023 ![]() Upon entering the league, Butler signed an endorsement deal with Adidas, where he rotated through myriad sneakers like the Crazy 8 and the Kobe 2. With most NBA players today, sneaker free agency is nearly as compelling of a conversation as basketball free agency. “They came in with a mentality like, “Just be yourself and work the way that you work.” “The Heat wanted me to be me,” Butler said. He has an alpha personality that has led to some uneven relationships with former teammates, Karl-Anthony Towns and Andrew Wiggins in Minnesota, but he has finally found a comfortable new home in Miami. He’s been on an All-NBA teams six times, and he has been to the playoffs seven out of his nine seasons in the league. Battlefield v release date8/8/2023 ![]() But the version that shipped a few weeks ago is a generic and unremarkable PvE experience with none of that promise. Combined Arms was originally intended to be much more ambitious, with a dynamic mission generator and a ramping difficulty that made squads make the tough choice to extract with whatever assets they already had or see the mission through to the end at the risk of losing it all. Practice Range and Combined Arms are ultimately onboarding-centric experiences meant primarily for newcomers that should have been included at launch the fact that they weren’t reinforces the idea that this game was rushed to market to capitalize on holiday spending even though it wasn’t ready. In the aftermath of that controversy, DICE is still slowly chipping away at its to-do list of previously promised features, none of which are capturing the imagination of its passionate player base. Instead, the entire player base was subjected to a rollercoaster of rushed hotfixes that wildly altered the gameplay. However, there was one small problem – Battlefield V inexplicably shipped without the Community Test Environment, which has been a series staple since Battlefield 3. When the contentious TTK-TTD fiasco dominated the fan feedback loop in the weeks following launch, DICE could have explored a fix the way most modern games do – on a test server. Battlefield V also had its own easily correctable problem. Battlefield 3 launched on PC without an in-game server browser thanks to EA’s stubborn insistence players use the browser-based Battlelog. Bad Company 2 inexplicably split squads of friends during matchmaking. With seemingly each new Battlefield game, the studio infamously reintroduces problems it already solved in previous games. ![]() Playing catch-up isn’t necessarily a new thing for DICE. Fast-forward three months, and we’re still waiting for the full slate of promised features, let alone any meaningful additions that evolve the game in interesting ways. Although the gorgeous graphics impressed and DICE made many smart, smaller evolutions to the classic squad-based play, the litany of missing features made the game feel like an under-construction early access title rather than a fully featured experience on par with previous entries in the series. Grand Operations only made its way into the Day One feature set after fans busted out the torches over news that the first major feature EA touted in its pre-release press blitz was going to come out shortly after launch and EA was forced to backpedal.Īs I said in my review, this incomplete picture made Battlefield V a tough game to evaluate on launch day. As crazy as that sounds, it almost didn’t ship with that mode either. ![]() ![]() When the November 15 release date arrived, only one of these promised features – Grand Operations – was included in its intended state. And not one to be left on the sideline while battle royales took over the world, Battlefield V would also have its own unique take on the popular mode, dubbed Firestorm. A new “Company” system would give players a greater level of flexibility for tweaking and saving class/vehicle loadouts for any battle scenario. A cooperative mode called Combined Arms would provide an evolving PvE destination. The evolution of the fan-favorite Battlefield 1 mode Operations, dubbed Grand Operations, would add a new array of objectives to the playlists to keep the evolving battles fresh. During the lead-up to the Battlefield V launch, DICE laid out the many ambitious ways it hoped to evolve the first-person shooter series. ![]()
